What is Hemorrhoids?

Hemorrhoids, which are frequently seen in our country as in the whole world, can lead to pain and rectal bleeding and impair the quality of life. It is a health problem that is usually hidden by patients because it occurs in the breech region. This disease, which can be treated with drug therapy or endoscopic interventions if it is noticed at an early stage, is usually concealed by the patients, and can only be referred to a doctor at the stages that require surgical intervention.

Hemorrhoids are the vessels in the rectum (the last part of the large intestine near the anus) or in the anal canal. This can cause complaints when swelling-dilation occurs in the vessels. Due to the enlargement of these vessels, vessel walls become thinner and bleeding is observed.

What Types of Hemorrhoids?

There are two types of hemorrhoids according to their location. External hemorrhoids are covered with sensitive skin and are located close to the anus. External hemorrhoids usually do not cause pain unless they are thrombosed. Internal hemorrhoids are enlarged cannons located in the rectum just above the anal opening. They can expand and hang out of the anus.

How do you know if you have internal hemorrhoids?

Even if you have internal hemorrhoids, it may not cause any complaints. If it presents a symptom, it most often presents as painless rectal bleeding. In addition, itching, pain-breech fullness feeling or hanging out of the tissue can be felt in the form of stool.

What are Internal Hemorrhoid Stages?

Stage I: Usually bleeding, but not hanging out of the anal canal.

Stage II: Although it hangs from the anal canal during twitching or stooling, it automatically returns to its original position.

Stage III: It hangs out of the anal canal spontaneously or straining and requires only a finger aid to its former position.

Stage IV: Chronically sagging hemorrhoids that cannot be reduced to their former position.

What Causes Hemoroide?

Chronic constipation or diarrhea

Pregnancy

Obesity

Hereditary

Over-strain during bowel movements

Spending a long time in the toilet

How can hemorrhoids be prevented?

Perhaps the most important treatment of hemorrhoids is the prevention of development. For this;

Drink plenty of fiber and adequate water; necessary for regular bowel movements.

Stool-softening drugs may be required for those with constipation.

Prevention of diarrhea may be beneficial in patients with frequent bowel movement.

What Should You Do If You Have Hemorrhoids?

External hemorrhoids may be treated by a Gastroenterology specialist or General Surgery specialist.

They can be seen and recognized directly in the physical examination. In addition, the rectal part of the anal canal is examined with the finger called the rectal touch. Visible if the internal hemorrhoids hang out or can be detected in the rectal touch. Rectosigmoidoscopy in patients less than 50 years of age and complete colonoscopy in patients over 50 years of age are performed by a Gastroenterology specialist and a definitive diagnosis of internal hemorrhoids is provided, and other pathologies that may be confused with hemorrhoids can be excluded.

How are hemorrhoids treated?

Most hemorrhoids can extinguish spontaneously or with some simple precautions. These measures

Despite these measures, complaints may or may not recur. In this case

Depending on the stage of your hemorrhoids, non-surgical endoscopic methods may be performed by a gastroenterologist. However, surgery may be necessary in the case of advanced hemorrhoids or unresponsive to these endoscopic treatments.

What are Endoscopic Treatments of Hemorrhoids?


Endoscopic treatment methods that are frequently applied in our clinic are; Rubber Band Ligation is Infrared Photocoagulation. Sclerotherapy is not preferred in our clinic because of its lower efficacy.


Rubber Band Ligation

The tape is placed around the hemorrhoids with a special apparatus placed at the end of the endoscopy. Thus, the blood flow of the hemorrhoid is cut off, the degenerated hemorrhoid structure, which extinguishes over time, is treated by the fall of the band within days. It is usually a painless intervention and after 24 hours the feeling of discomfort in the rectum disappears. Painkillers can be used in mild pain situations. Warm seating bathrooms also make this period more comfortable.

Infrared Photocoagulation

In this technology, a 15 volt tungsten-halogen lamp with a teflon polymer at the end

produces infrared light. Each hemorrhoid nozzle is irradiated three times for an average of 1.5 seconds and three times with the resulting heat. Although pain is less than band ligation, it is more effective in early stage (stage I) hemorrhoids.

sclerotherapy

During sclerotherapy, a special chemical agent is injected into the hemorrhoid tissue and the hemorrhoid tissue is destroyed. However, this treatment method is not as effective as the two procedures above.
