ANAL FISSURE
Fissure means crack, cleft. Anal fissure is caused by a longitudinal rupture of the anal mucosa due to the trauma of the solid stool. This pathology is one of the most important causes of painful defecation, bloody stool and constipation in children. Most of them recover spontaneously within a few weeks. Fissures that do not heal up to three weeks are also called chronic fissures.
WHAT ARE THE CAUSES OF ANAL FIXURE IN CHILDREN?
The hard stool passes through the anus, causing tears in the tissues. Changes in the consistency of feces with the use of some drugs, diarrhea very frequent defecation, interventions to the anus can cause fissure formation. Anal fissure may also be the first sign of inflammatory bowel disease. The anal fissure is often found in the posterior middle of the anus where the blood supply is impaired.
WHAT ARE THE CLINICAL FINDINGS AND FINDINGS OF THE ANAL FISSURE?
Pain and bleeding occur during defecation. Pain is a more pronounced finding. In babies who have not yet come out of the diaper, it causes crying while removing gas and feces. In older children, it is the most important reason for holding constipation and poop. Deepening anal fissure causes bleeding. In these children, fresh blood is transmitted to the toilet paper while streaking on the stool or cleaning the bottoms after defecation.
HOW IS THE DIAGNOSIS OF THE ANAL FIXURE?
On physical examination, a rupture leading to the anus can be seen, usually located on the posterior side of the anus. They are found in the front middle part of the baby. Infants with chronic constipation or anus finger examination may be multiple. After the anal fissure has healed, an outward skin protrusion (sentinel cell) may form at the edge of the anus. Especially if there is bleeding, this appearance can be mistakenly defined as “hemorrhoids.. Anal fissure may also be inside the anus. In this case, endoscopy is required for diagnosis.
WHAT IS HELD IN ANAL FIXURE TREATMENT?
Most anal fissures heal spontaneously. If there is constipation and painful poop, poop softener is given and diet treatment is applied. Internal anal sphincter spasm was dissolved by applying cream and anal fissure was healed. Surgery is required in cases that do not heal with medical treatment, but this is very, very rare in children. Surgical treatments include anal dilatation, sphincterotomy and fissurectomy.
WHAT IS HEMOROID?
Hemorrhoids, commonly known as hemorrhoids, are the anus vessels hanging out due to swelling. It is very rare in children.
WHY HEMOROID IN CHILDREN?
Constipation is the leading cause. Constipation will bring with it more pushing defecation. The pressure in the anus will increase and hemorrhoids will form. Obesity is another cause of this disease. Other causes include irritable bowel syndrome, inflammatory bowel disease, and portal hypertension.
WHAT ARE THE SYMPTOMS AND FINDINGS OF HEMOROID IN CHILDREN?
Itching around the anus, pain when pooping and bleeding in later periods are the main symptoms. The itching is caused by the release of mucus by the hemorrhoid tissue, which moistens the anus and its surrounding tissue. Pain and discomfort while pooping are other symptoms that force the child to undergo a troublesome process. There is also a protrusion in the anus. Bleeding from the anus is rarely seen in children. Bleeding is bright red.
HOW TO TREAT HEMOROIDINE TREATMENT?
Constipation is the most common cause in children. It is an important part of the treatment with sitting baths with warm water 2-3 times a day. The hemorrhoid area should not be irritated during toilet cleaning. After the toilet needs, the anal area should be cleaned with a wet and soft toilet paper. Pain relief and anti-hemorrhoidal creams should be applied to the anus area.
