Thus, pain, edema, swelling and inflammatory image regresses a lot.

2 - Pain Relievers

Pain relief is essential. Painkillers may be in the form of tablets, needles or creams. These soften anal spasm; relieves edema, swelling and pain. If necessary, a single dose of painkiller should be given daily for a day or two to completely relieve the pain. Pain should never be allowed, otherwise pain, spasm and ischemia, which again brings hemorrhoidal swelling.

These creams have antiseptic, vasoconstrictor, pain, edema and itching relieving, softening effects.

Cream selection; it varies according to the type of complaints, the degree of the disease, examination findings.

It causes serious complications with wrong cream usage. Acute, especially flaming conditions, especially; in all hemorrhoids; recovery may be delayed even after intervention.

For example, cortisone creams may adversely affect treatment or some of the creams in patients with damaged and bleeding fistula disease; If used for more than 2 weeks, it causes allergic itching and inflammation in the environment.

Again, if you need to use cream for a long time, no side effects, vaseline, cortisons-free creams should be preferred. In acute, thrombosed and intervening hemorrhoids, antibiotic creams should also be added to the treatment as they are usually infected.

4- Stool softening
The softening of the stool and the reduction of the number of defecation, the formation or exacerbation of hemorrhoids and fissures, the type of foods, stiffness or stickiness of the feces, constipation or frequent defecation and sometimes diarrhea has negative effects; diarrhea and especially constipation should be corrected; the exit to the toilet must be programmed and the layout determined.

Types of Hemorrhoids and Treatment Methods

1st Degree Hemorrhoids: If the hemorrhoid nozzle can be seen in an upward position and not coming out of the anus but entering through an anoscope, it is a 1st degree hemorrhoid.

It manifests itself only by bleeding. These nozzles are generally painless and are less than 1 cm in size, stretched and thin-walled, ready to bleed internal nozzles and do not handle.

Treatment: 1st degree hemorrhoids can be treated by medical methods such as rubber banding, sclerosing drug injection and laser treatment, and surgery is not required. Sometimes only ointment, proper diet, warm water sitting bath and rest can be enough. However, preparatory reasons should be investigated and treated separately.

2nd degree Hemorrhoids: These are painless, 1 - 3 cm diameter Hemorrhoidal breasts that come out of the anus when they are pushed to the toilet during the defecation and disappear into the anal canal when they stand up, usually bleeding in the form of sprayed or rapid drops. Bleeds can sometimes take a break for a month or two, sometimes for months, with a small amount of time at each toilet exit. And in time, it surely causes anemia, and sometimes it may flare up and grow and become locked out of the anus and become urgent.

Treatment: Second degree hemorrhoids do not require surgical treatment. Binding is treated with sclerosing drug injection. In addition, appropriate diet, astringent and antibiotic ointments, warm water sitting baths and painkillers should be added to the treatment.

3rd Degree Hemorrhoids: Internal hemorrhoid breasts easily go out of the anus, often cause pain with clots and edema, wounds and bloody discharge in the form of inflammation and aphthae; unless it is pushed in.

Treatment: Depending on the situation, treatment is started by medical and conservative methods; very few cases require surgical excision. The most ideal method is rubber band or laser application.

4th degree Hemorrhoids: For years of chronic constipation in cases of old internal and external hemorrhoids hanging down collectively, as a result of sitting in the toilet or seat, breast, large, painless, watery, wet masses outside the anus is to settle around all around. Chronic 4th degree patients have poor cleansing and persistent mucus and inflamed rashes, pruritus, and minor but frequent bleeding problems. Pain is felt when sitting on the breasts.

Treatment: Most of them benefit from medical and hygienic measures such as good medical treatment, constipation-free diet, regular toilet habits. In some, rubber band or sclerotherapy, laser application may be sufficient; however, surgical treatment is required in most cases. If there is an obstacle to surgery, it will continue the lifelong, conservative medical methods and it is always necessary to consume plenty of water, plenty of vegetables and fruits.

IV: Stage hemorrhoids: hemorrhoids

Hemorrhoids can be a pre-symptom of some diseases. Hemorrhoids may occur as a first symptom in diseases such as liver cirrhosis or portal hypertension (an increase in the pressure of an important vein called intra-abdominal portal vein) which may occur with intense alcohol consumption, liver diseases and hepatitis B (jaundice transmitted by blood or sexual contact). General Surgery Specialist Op.Dr.Kadir Uskuay gave information on the subject.

Hemorrhoids, also known as hemorrhoids, are known and treated for at least 4000 years. However, the disease and its treatment have recently become understandable.

Hemorrhoids are present in one in 2-3 people in the community. It is seen equally in both men and women. The incidence increases in middle ages and decreases after 65 years. It is the disease that causes the most disability in community life.

Hemorrhoids are tissues that normally exist and serve to detect feces, feces and gas retention, and prevent breech irritation in defecation. Disease occurs as they loosen and expand over time and hang out from the rectum.

Familial predisposition, constipation, long sitting in the toilet, overstressing, obesity, diarrhea and pregnancy are the factors affecting the formation.

Hemorrhoids in the area where we can touch by hand are external, and those coming out of the rectum are internal hemorrhoids.

Internal hemorrhoids also present in four degrees.
First degree: Bleeding may occur.
Second degree: The toilet comes out of the nipple, and when it is done, it is pulled in.
Third degree: You need to push the nozzles that come out after the toilet by hand.
Fourth degree: Hemorrhoids are constantly outside the breast, do not enter.

Hemorrhoids are most likely to be confused with many diseases, particularly cancer. For this reason, anyone with the smallest complaint from this region, who has bowel cancer in their family, should consult a physician and clinic who are experts in this field and who can provide the diagnosis and treatment of this disease. Examination is life-saving. Diagnosis and removal of the structure that we call polyps in the intestine before the cancer begins, prevents cancer without the need for surgery. Hemorrhoids can be treated in 10 minutes without the need for surgery.

The treatment of hemorrhoids is very easy in expert hands, painless, without anesthesia, without any preparation, without breaking from daily life and without risk.

Treatment is planned after an absolute examination of the last 20 cm of the hernia and the intestines (especially to investigate the presence of another disease).
Drug therapy is tried in newly started hemorrhoids. The patient relaxes and suffers if the disease does not recur.

Non-surgical treatment should be preferred.
It is the most commonly used band ligation method. It is applied within 5-10 minutes, no preparation is required, no anesthesia is required. The patient may not even notice that the treatment is performed in the hands of the expert person. There is no possibility of complications. The patient returns to his daily life and work after the treatment.

Another method is drug injection. Usually alone is insufficient. Can be used together with tape method.

When the disease reaches the fourth stage, surgery is inevitable. Surgery requires hospitalization and general anesthesia. It is very painful, the healing lasts 15-45 days and there is a small chance of complications.

Do you know? It is one of the most common diseases. Hemorrhoids develop in more than half of the population, usually after the age of 30. Millions of people around the world suffer from this disease. This problem can be solved painlessly with current surgical treatment methods.

What is Hemorrhoids?

Hemorrhoids are vascular tissue cushions around the breech. Hemorrhoids swelling of these vessels
Evident.

What are the different types of hemorrhoids?
Hemorrhoids in the rectum are divided into two as internal and external.

Internal hemorrhoids are in the rectum. You are only aware of their presence when they sag, bleed, or produce slimy discharge. Painless bleeding and palpable mass during or after defecation are the most common findings. With the examination of your doctor, it is easily understood whether you have internal hemorrhoids.
External hemorrhoids are small bubbles at the outlet of the breech. They are usually painless. However, blood clots may form in these hemorrhoids, in which case they turn blue and cause severe pain; at other times it has normal skin color. External hemorrhoids that cause complaints may cause swelling, burning and pain in the breech and rarely itching.
Causes of hemorrhoids
Although the exact cause is unknown, pressure increases in the vessels in the breech region and enlargement of the hemorrhoids occur due to the standing position.
Other factors that contribute to the formation of hemorrhoids;
· Age
· Chronic constipation or diarrhea
· Pregnancy
· Familial
· Obesity
· Excessive force during defecation, straining
Excessive use of laxatives or enemas to relieve constipation
· Spending more time (more than 3-5 minutes) in the toilet (newspapers, magazines, books)
reading, etc.)
Whatever the cause, the vascular cushions in the anal area are stretched. The veins here
dilates, thins and bleeds. If pressure and pressure persist, these cushions come out and show themselves as a handled mass if no action is taken.

If you have complaints such as itching, pain, swelling or irritation, you should not think that you have hemorrhoids directly. You should consult your doctor for an examination to make sure that you do not have a more serious problem, such as polyps, colitis, or cancer. In case of bleeding from the rectum, you should consult your doctor. Itching is rarely due to hemorrhoids.

What is appropriate treatment?
Appropriate treatment includes both surgical and non-surgical treatment.

Non-Surgical Treatment
Often, changing the nutritional habits creates miracles. In the first place, we advise patients on their eating habits; a diet rich in vegetable fibers, low cholesterol and at least five cups of water per day to advise.
Drug Treatment of Hemorrhoids
· Take high amounts of fiber food and drink plenty of water. At the same time, you need to reduce the intake of animal fat.
· Exercise daily with your doctor's approval. These exercises are brisk walking,
whether you like cycling, running, swimming or doing
it may be any activity that it endorses.
· Remove defecation needs once a day in the morning. Do not go to the toilet with every false warning.
· Avoid overstressing during defecation, because with overstressing, you will unnecessarily force the breech and this will increase your complaints due to hemorrhoids.
· You can apply a pomade written by your doctor to the hemorrhoid area or suppository
You can use.
· Sitting baths with 10 minutes of warm water during the day will reduce your complaints.
These measures will reduce your complaints in 2 to 7 days. Handled in 4-6 weeks
The breast will shrink. Thrombosed hemorrhoids (blood clot)
) can be improved. Therefore, if you consult your physician under local anesthesia
will help you with intervention.
Ligation - Rubber band treatment is very effective in the treatment of internal hemorrhoids that have come out correctly. With the application of the rubber band applied above the hemorrhoid pakes, the blood supply is stopped and within a few days this pake goes out and the band falls off. In 1-2 weeks, the patient's complaints regress. It is sometimes necessary to repeat this method.
Etching (Coagulation) - Using a light source, a small burn is created above the hemorrhagic cake, thus preventing hemorrhage from bleeding and reducing it to its normal size. It is used in hemorrhoid packs with bleeding. It is not applied to hanging out.
Injection - In this process, a fluid is injected above the swollen hemorrhoid to reduce it, more effective in hemorrhages that do not sag from the rectum. The procedure may need to be repeated every two to three years.
These methods are used only for internal hemorrhoids. If used in the treatment of external hemorrhoids, they cause severe pain.

Surgical treatment
Advanced hemorrhoids are usually described above, which can be applied in the outpatient clinic
they do not respond to methods.
· Blood clot (thrombus) with severe pain in the external hemorrhoids,
· Internal hemorrhoids that do not respond to band application method,
· In case of hemorrhoids that come out of the rectum and cannot be placed in,
Surgical treatment is required in case of bleeding from hemorrhoids.
Hemorrhoidal disease is treated by cutting out the pouches that bleed out and hanging out with special or classical instruments or hanging them with the stapler method. These surgical procedures can be performed under local anesthesia with a few days' home rest without hospitalization.

Do not forget:
Hemorrhoids are common and do not threaten the life of the affected person. However, it should be kept in mind that you may also have an important medical problem that you may overlook until your doctor is diagnosed with hemorrhoids. Generally, hemorrhage-related complaints are reduced by changing dietary habits, focusing on fiber foods, avoiding fatty foods, and increasing daily drinking water.
When surgical treatment is needed, it should be remembered that there are many new methods to reduce pain.

After Treatment:
Symptoms:
In the first 36-48 hours, you may feel mild to moderate pain, blunt pain, or you may not actually feel anything. If your illness is mild, you can use a simple painkiller. Do not use aspirin or blood thinners with aspirin, as these drugs dilute the blood and trigger bleeding. If your pain is severe, your doctor will prescribe medication for you. A 10-15 minute warm sitting bath reduces your discomfort. If hemorrhoid treatment is applied surgically, 8 hours after the operation, the external dressing can be removed and a sitting bath can be made in warm water. You may have a small amount of discharge from the wound for several days after the operation, you may bleed. There is no need to dress for this discharge, the wound heals within 10 days.

Nutrition:
After treatment, it is important to ensure that your defecation is regular and smooth. Eat plenty of foods rich in fiber and drink plenty of water (6–8 glasses a day). If your doctor has recommended that you use a fiber-rich nutritional supplement, continue. Caffeine has a constipating effect. Therefore, you should reduce the use of coffee, tea, chocolate and cola.

Physical Activity:
You can resume normal physical activity. You can drive your car immediately after treatment,
you can climb the stairs and continue your normal movements.

Conditions to be Concerned:
If any of the following conditions occur, consult your doctor.
· Pain that persists for three days

Increased pain within a few days after treatment
· Painful swelling in the breech
· Fever, chills tremor
· Difficulty urinating
· Severe constipation (no defecation for 3 days)

Diarrhea (watery defecation more than 3 times in 24 hours)

Visible if the internal hemorrhoids hang out or can be detected in the rectal touch. Rectosigmoidoscopy in patients less than 50 years of age and complete colonoscopy in patients over 50 years of age are performed by a Gastroenterology specialist and a definitive diagnosis of internal hemorrhoids is provided, and other pathologies that may be confused with hemorrhoids can be excluded.

How are hemorrhoids treated?

Rubber Band Ligation

The tape is placed around the hemorrhoids with a special apparatus placed at the end of the endoscopy. Thus, the blood flow of the hemorrhoid is cut off, the degenerated hemorrhoid structure, which extinguishes over time, is treated by the fall of the band within days. It is usually a painless intervention and after 24 hours the feeling of discomfort in the rectum disappears. Painkillers can be used in mild pain situations. Warm seating bathrooms also make this period more comfortable.

Infrared Photocoagulation

In this technology, a 15 volt tungsten-halogen lamp with a teflon polymer at the end

produces infrared light. Each hemorrhoid nozzle is irradiated three times for an average of 1.5 seconds and three times with the resulting heat. Although pain is less than band ligation, it is more effective in early stage (stage I) hemorrhoids.

sclerotherapy

During sclerotherapy, a special chemical agent is injected into the hemorrhoid tissue and the hemorrhoid tissue is destroyed. However, this treatment method is not as effective as the two procedures above.
